Insurance

We participate with the majority of insurance carriers who do business in southern New Jersey. Participation means that we will bill your insurance carrier for the services we render and follow their payment guidelines. Every insurance carrier has different rules based on the type of contract you have. Once your claim is processed, you could be responsible for a co-insurance and or deductible or any unpaid co-pays.

Some managed care plans require a referral from your primary care doctor. This referral may be a paper form, which you are required to pick up at your primary care physician's office and bring with you for your appointment.

Some referrals are electronic. Your primary care physician will enter the referral online and our office will retrieve it. It is very important that your primary care physician issues the referral and, if it is a paper referral, you present it at your office visit.

With a proper referral, we can expedite your care and schedule any necessary tests or surgical procedure without delay.

If your insurance requires a co-payment, we will collect what we believe to be the appropriate co-payment at the time of service. There are situations in which the co-payment may not be collected at the time of service and you could be billed at a later date. Usually this happens when your plan has changed co payment amounts, but you have not yet received your new ID card.

Some insurance companies also do not post the co-payment amount on the ID cards. There are other times, particularly with our noninvasive vascular lab testing that we defer collecting a co payment because your insurance carrier may not make it clear to us that we are permitted to collect for that test.

Worker's Compensation Visits

If you are considering making an appointment for a problem or injury that either is definitely related to your job or you believe it is related to your job, it is imperative that you file a claim with your employer before making an appointment. Some employers' Worker's Comp Insurances have rules that require you to see a specific physician who will direct your care and send you to a specialist only if needed. Please consult your employer to be sure that your company's guidelines are followed.

When you make your appointment to be seen by our surgeons, you will need to provide the following information:

  • Claim #
  • Full name, address and phone number of the workers compensation insurance company
  • Worker's Comp company Contact Name, phone # and fax #
  • Date of Injury

Motor Vehicle Accident Visits

If you are considering making an appointment for a problem or injury that either is definitely related to a motor vehicle accident or you believe it is related to a motor vehicle accident, it is imperative that you file a claim with your motor vehicle insurance before making an appointment.

Some Automobile/Motorcycle Policies have rules that require you to see a specific physician who will direct your care and send you to a specialist only if needed. Please consult your Automobile/Motorcycle Policy Representative to be sure that your policy's guidelines are followed.

When you make your appointment to be seen by our specialists, you will need to provide the following information:

  • Claim #
  • Full name, address and phone number of the motor vehicle insurance company
  • Motor Vehicle Insurance company Contact Name, phone # and fax #
  • Date of Accident

Self Pay Patients

Payment in full is expected when your office visit is rendered. Payment arrangements may be made for certain procedures. Please speak with our billing department regarding our payment plan.

Charity Care

Charity Care is funded by the State of New Jersey and provides limited compensation to hospitals for the services they render. It does not compensate physicians for the services they render either in the hospital setting or in the office. Our billing department would be happy to work with you to set up a payment plan in this case.
